Yes, we work with your medical team closely and work with prescriptions from your physician, physician assistant or nurse practioner 

the prescription should indicate:

your name

Your date of birth

diagnosis related to your automobile accident injuries

the type of service requested.  

any precuations/restrictions 

This is decided on a case-by-case basis depending on the primary health insurance policy. 

you should ALWAYS feel you can contact your therapist directly if you have questions about your care.  However, if there are any issues you would like to discuss that you would feel more comfortable talking with another representative from Kalamazoo Functional Rehab Team, you will be provided with the contact information for the Lead Therapist (quality control) or Director of Operations (safety and compliance).  

Kalamazoo Functional Rehab Team is committed to a "people first" approach to services.  We want you to be in the setting you will be most successful and will help with any transitions or collaboration needed if a transfer of services is needed. 

We will never bill you directly for services rendered.  Your insurance coverage will be verified prior to the start of services and if there is an unpaid balance - KFR Team will work with your insurance/adjuster but will never bill you for the difference or report you to credit agencies.
