KFR Team wants to help teach clients the tools they need for long-term possible results. In other words, we want clients to feel they are able to continue or maintain their progress with rehabilitation outside of skilled services.
To facilitate coordination of care, KFR Team wants to provide communicate the findings of assessments to other team members (case managers and providers) in a timely manner according to the plan of care.

Response to referrals is important to ensure clients have access to services without delaying care. KFR Team is working to ensure swift response to referrals.
KFR Team is working to decrease time spent documenting by implementation of electronic documentation.
