Performance Improvement Initiatives 2025

Effectiveness Of Services

KFR Team wants to help teach clients the tools they need for long-term possible results. In other words, we want clients to feel they are able to continue or maintain their progress with rehabilitation outside of skilled services.


Efficiency of Services

To facilitate coordination of care, KFR Team wants to provide communicate the findings of assessments to other team members (case managers and providers) in a timely manner according to the plan of care.


Client Satisfaction with Services

KFR Team wants clients to feel they would recommend our services to a family member or friend


Satisfaction of Referral Sources

KFR Team hopes to be a preferred provider of therapy services.


Access to Services

Response to referrals is important to ensure clients have access to services without delaying care. KFR Team is working to ensure swift response to referrals.
