Rights and Responsibilities

Patient Rights


  • As a consumer you have the right to be treated with respect during all aspects of your care.
  • You have a right to receive services no matter what your race, religion, sexual orientation, age, or disability.
  • You should expect people working with you to never intentionally say hurtful things to you.
  • You can expect the people working with you to do their best to maintain your modesty and dignity.
  • Expect that the people working with you will never expose you to physical or sexual abuse or any intentional harm.
  • You can expect all information to be kept confidential unless you agree to release it.  
  • You will have a treatment plan, made by you and your clinician and that you agree to work on. 
  • You always have the option to refuse participation in any kind of survey or research.
  • You should expect your opinions about your care to be heard and used to improve our services.
  • You may seek advocacy help if you do not believe you are being treated fairly.
  • You have the right to ask questions about anything you don't understand.


Patient Expectations

As a consumer of services through Kalamazoo Functional Rehab Team, you would have the responsibility to: 


  • Be courteous to staff 
  • Report your strengths, needs, abilities and preferences to your clinician as honestly and completely as possible
  • Ask questions about anything you don't understand.
  • Inform your clinician should you have special needs
  • Actively participate in your treatment and in meeting your goals to the best of your ability
  • Inform your clinician if you wish to discontinue therapy. 
  • Participate in therapy without alcohol or illegal/unprescried drug use.

  • Care should be respectful  and compatible with cultural health beliefs and consumer's preferred language.
  • Written information provided will be in consumer's preferred language.
  • Care providers will provide qualified care regardless of your socio-economic status, race, religious belief or gender orientation/sexual preferences.
