- A Valid prescription for services provided from a licensed physician, physician assistant or nurse practitioner caring for you is needed.
- The services should be to address deficits related to conditions resulting from your auto or work-related accident.
- If you have a case manager assisting with your claim, Kalamazoo Functional Rehab Team will coordinate services through the case manager.
- A member of Kalamazoo Functional Rehab Team will verify Insurance coverage with your adjuster. (We currently accept aut0 insurance and pre-authorized workman’s compensation, we will bill Blue Cross or Medicare if benefits are coordinated). If you have a limited coverage auto policy (“capped”) we will determine acceptance case-by-case basis.
- A member of Kalamazoo functional Rehab Team will reach out to you for scheduling an initial visit.
- Either before or at the initial visit, you will be asked to review/sign consent and intake forms. These forms involve consent for billing, treatment, release of information, photography, acknowledgment of privacy rights, emergency information, risk assessment/screening and personal/cultural history.
- You will be provided with a folder that contains information about Kalamazoo Functional Rehab Team and its services, your rights and responsibilities as a patient, the rehabilitation process and the process to file a grievance should you have one.
- Once consents/intake information is reviewed, the clinician will continue the evaluation – this may be physical (movement as well as skin) and cognitive and may involve you demonstrating how you move around in/access areas of your home.
- After evaluating you, the clinician will discuss their impressions and ideas for treatment with you and help establish goals that you both agree to (short and long term) – this plan will be communicated to your referring provider and case manager (if applicable)
- The rehabilitation process should focus on providing you with the tools to empower you to continue your progress on your own or with the help of reliable caregivers. This may include home exercises/activities for you to do between sessions.
- Caregivers/Family members may be expected to assist or be present during sessions.
- If equipment is needed, this will be coordinated through your case manager, if applicable.
- Periodic progress reports will be completed and communicated to your referring provider and case manager.
- Once you have reached a point where you are either at your maximum rehabilitation potential OR you have the ability/knowledge and tools to progress on your own, the clinician will discontinue formal therapy services.
- We encourage OPEN communication throughout this process to adjust goals or the plan of care as needed.
