Home Needs Assessment Services

We offer home accessibility and equipment needs assessments by PT or OT.  Our therapists will coordinate with your case manager and/or accessibility coordinator to do a thorough assessment of your home and your specific needs for accessibility to make recommendations for accessibility needs and/or equipment that will facilitate your mobility, safety and/or independence in your residence.  Once our recommendations are made, a contractor is likely chosen to assist with determining construction options that can meet the needs outlined in the assessment. The accessibility coordinator/case manager then works with your insurance provider to determine options/feasibility of modification or equipment acquisition.
